Position Summary: Under the direction of the Chief Behavioral Health Officer, the Licensed Clinical Social Worker is responsible for performing psychological assessments and providing brief evidence-based treatment using the primary care behavioral health model to adults, older adults, children, and their families in a primary care setting. This licensed clinician works closely with primary care providers to develop and implement sound treatment plans, responds to and provides crisis intervention services when needed, and ensures behavioral health services are delivered in a culturally appropriate manner, in accordance with California laws, professional standards, agency guidelines, and ethical guidelines. Essential Functions of the Job: Evaluate, triage, and provide evidence-based, brief psychological/behavioral health interventions to patients with various diagnoses. Provide consultation to clinical staff and primary care providers based on the Primary Care Behavioral Health Consultation model, including feedback and treatment recommendations. Perform brief assessments of patient needs and develop negotiated care plans with patients and clinicians. Maintain accurate and timely documentation in electronic medical records and produce evaluation/treatment reports as required. Evaluate and revise care plans as needed, documenting changes and communicating with the multidisciplinary team. Perform crisis interventions, including suicide assessments, and make safety recommendations. Collaborate with the Chief Behavioral Health Officer to inform healthcare providers about mental health issues in the medical setting. Assist in implementing and administering the ESBIRT program for substance abuse prevention and treatment. Participate in meetings, case reviews, and committees as required. Meet productivity expectations according to clinic guidelines. Perform triage, assessment, and stabilization in crisis situations, including telephone and mobile crisis services. Perform other duties as assigned. Additional Duties and Responsibilities: Adhere to departmental policies, quality assurance, safety, and infection control standards. Organize, prioritize, and exercise sound judgment within responsibilities. Be culturally sensitive and appropriate when working with underserved communities. Establish and maintain contact with community resources for continuity of care. Build effective relationships with clients, families, staff, and the public. Seek guidance from supervisors when necessary. Engage in professional development through education, literature, and workshops. Provide outpatient services on school campuses and satellite clinics as requested. Supervise interns and unlicensed staff as appropriate. Work with providers to establish integrated, team-based care services. Conduct presentations, trainings, and lectures on behavioral health topics. Assist in developing SBIRT train-the-trainer models for primary care teams. Monitor progress toward contract goals to ensure compliance. Perform other related duties as assigned. Job Requirements: Experience Required: Sensitivity to multicultural concerns and psychotherapy processes. Preferred: 2 years of experience in community health/mental health settings. Preferred: Experience in Behavioral Medicine/Health Psychology. Education Required: Successful completion of a terminal graduate program in a psychotherapy-related field: Ph.D./Psy.D. in Psychology or equivalent from an accredited institution. Preferred: Experience in Behavioral Medicine/Health Psychology. Licenses Required: Psychologist license (PsyD/PhD) issued by the state of California. Skills and Knowledge: Bilingual English/Spanish with excellent communication skills. Knowledge of rehabilitation, recovery, behavioral medicine, health psychology, and substance abuse interventions, including IMPACT and SBIRT models. Cultural sensitivity and respect for diversity. Ability to organize, prioritize, and exercise independent judgment. Effective communication and relationship-building skills. Equipment and Conditions: Proficiency in MS Office (Word, Excel, Outlook, PowerPoint). General office environment; frequent sitting, occasional standing, walking, lifting up to 10 lbs.
