Sentara Health is seeking to hire a qualified individual to join our team as Care Coordinator! This is a temporary, full-time position with remote work arrangement limited to Virginia residents only. The position pays $24/hour and is expected to last a minimum of 3-6 months, with standard working hours Monday through Friday, 8am - 5pm. Responsibilities include performing a variety of casework duties and providing case management services to patients, families, and designated caregivers. The role requires developing, participating in, and monitoring multidisciplinary collaboration of services to patients where appropriate, assisting adult patients and their families with personal and environmental difficulties associated with medical conditions, including terminal illnesses. Education requirements include a Bachelor's degree in Health & Human Services or related fields from an accredited U.S. institution. No specific certification or licensure is required. Experience required includes 1 year of Long Term Care experience; health plan and Medicaid experience preferred. Strong communication, assessment, and advocacy skills are necessary to support patients through various stages of care.
